Case Management



Case management is a process devoted to the coordination, rehabilitation, care and support of people with complex, clinical needs. It aims to facilitate their independence and improve their quality of life; whilst acknowledging safety issues’.
British Association of Brain Injury Case Managers (BABICM) 2009.

It is important that the individual and their families identify a case manager in whom they have trust and confidence and with whom they can develop a good professional relationship. As case managers come from different professional backgrounds and have their own particular areas of expertise, the specific skills, knowledge and experience of the case manager should be determined prior to engagement.

Once engaged, a case manager will assess the needs of the individual and their family and identify relevant and cost effective resources which can be made available to them in order to obtain maximum benefit. A plan is then prepared and implemented to create a regime that is tailored to the individual. It is important that the case manager provides support and advocacy for the individual and develops an appreciation of their illness or injury and how this may impact on their lives and the lives of the family and those around them.

They will develop rehabilitation goals with the individual and be responsible for the recruitment and training of support staff and the establishment of the necessary levels of care required either at home or in supported living environments, and to liaise with external agencies to help obtain the funding necessary to meet the individual’s needs in the long term.
